CYP Case Manager – Associate

A forward-thinking, quality focused independent case management company, specialising in catastrophic and complex injuries is looking for a Clinical Case Manager to work with children and young people, with at least 2-years’ experience in a similar role.

What you’ll do as Clinical Case Manager?

You will provide support to children and young people with a range of needs including traumatic and acquired brain injury, spinal cord injury and amputation. You should be highly competent and dynamic in developing client-centred rehabilitation packages.

Some elements of the role can be completed remotely; however, this position also requires regular ‘real world’ face-to-face meetings with clients and their families, support workers and other members of the multi-disciplinary teams as well as some occasional attendance at the main office in Ipswich. We welcome applicants who live in our region, namely East Anglia, the Home Counties or London.

Staff development is supported with in-house training days and mentoring.

Main Responsibilities:

Coordinate services and resources for clients, ensuring timely access to necessary support.
Maintain accurate and up-to-date case records and documentation.
Monitor client progress and adjust rehabilitation plans as needed based on ongoing assessments.
Advocate for clients by connecting them with appropriate resources and services.
Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
Conduct regular follow-ups with clients to evaluate satisfaction and effectiveness of services provided.
Provide emotional support and guidance to clients and their families throughout the case management process.

Skills and Experience:

A background in Occupational Therapy, Nursing, Physiotherapy, Speech and Language Therapy, Social Work or Clinical Psychology.
Registration with HCPC, NMC, Social Work England.
At least 2-years’ experience as a CYP Clinical Case Manager
Strong organisational skills with the ability to manage multiple cases effectively.
Excellent communication skills, both verbal and written, for clear interaction with clients and professionals and effective report writing.
Empathy and understanding towards clients’ needs and challenges.
Problem-solving skills to navigate complex situations and find suitable solutions.
Proficiency in IT for documentation and reporting purposes.
This position is ideal for individuals who are dedicated to making a positive impact in the lives of others through effective case management practices.

We are open to applicants who wish to have the benefits of working on a self-employed basis, either full-time or part-time. Salary is dependent on skills and experience and will be discussed at interview.

Christa came to visit me numerous times in the Royal Free Hospital. Having assessed my needs, she organised repair of my wheelchair, hospital bed on my return home. Organised private physiotherapy and stay in care home. Once home, she came with me to private therapist in Harley Street.

Before I started working with Ben I was struggling with the everyday tasks e.g. getting myself dressed, walking any distance, lifting, getting in and out of bed, bending. Within six months, Ben and his team enabled me to do all these things and return to work gradually, which resulted in me being able to do my pre-injury job.

My life began to significantly change. Ben treated me with the upmost dignity and respect. He coordinated a team of professionals experienced in working with neurological disorders, especially with people who had experienced an accidental severe traumatic brain injury or through illness. Ben was very helpful and promptly organised a good local physiotherapist for me. There I was given exercises specific to my problem, along with acupuncture and massage. This was invaluable, as nothing had been offered to id my recovery by my local hospital. In addition, Ben organised a six-month gym membership at my local leisure centre.

I always found Ben easy to talk to and he regularly called to check on my progress. I would not hesitate to recommend Ben Holden td, who gave me a very friendly and professional service.
